You will need to carry separate ID cards for Medical and
Prescription Drug benefits.
If you are enrolling for the first time, you will receive your UnitedHealthcare
and PharmaCare cards in the mail prior to January.
Guardian Dental, St. Louis Dental and Vision Benefits of America do not provide
ID cards. You do not need an ID card to access benefits in either plan. Simply
confirm that your provider is part of the appropriate network when scheduling
your appointment.
