Dental coverage helps you and your family with the costs of maintaining good dental health and treating dental disease or injury.

You have a choice of two plans.

• St. Louis Dental/Dental Source, a Dental Maintenance Organization (DMO) Plan

This plan works like a dental HMO. When you enroll with St. Louis Dental you must choose a Primary Care Dentist (PCD) for you and your family. If you do not submit a valid PCD when you enroll, St. Louis Dental will send a new member notification package and request that you select a PCD. You must use a network provider to receive benefits. There are no out-of-network benefits.

• Guardian Dental, a Preferred Provider Organization (PPO) Plan. This plan is funded by the district and administered by Guardian Insurance Company.

Guardian Dental Insurance offers you the option to visit any dentist or specialist you choose or a DentalGuard Preferred network dentist, any time you need care. If you do visit a network provider, you'll receive the highest level of benefits and save on out-of-pocket costs. After you pay the applicable annual deductible, the indemnity plan pays a percentage of dental expenses up to an annual maximum. You can search for Guardian Dental Insurance providers at Guardian Dental Provider Search. When prompted, select the correct coverage option under PPO Networks -- the DentalGuard Preferred.

Your personal enrollment worksheet lists the options available to you, along with each option's cost per pay period. For more information on these two plans, see the dental plan comparison chart. You do not need an ID card to access benefits in either plan. Simply confirm that your provider is part of the appropriate network when scheduling your appointment.
