You must enroll for benefits during the
Open Enrollment period – November 9 through
November 24 at Midnight, CST. If you don’t
enroll during this period, you will receive default benefits – the
same plan and coverage level you had in 2006. (See If
You Do Not Enroll for more information.) The chart
below provides more details about the coming weeks.

| Confirmation statements arrive at your home |
Early December |
• A personalized confirmation statement
listing your benefits for 2007 will be mailed
to your home.
• If your confirmation does not reflect your elections for 2007, call a
Customer Service Representative (CSR) on the SLPS Benefits Call Center phone
line at 1-866-345-SLPS (7577).
• Call before December 31, 2005 to correct any errors or discrepancies with
your confirmation statement.
