You may enhance your District-provided
Basic Group Term Life Insurance by electing Supplemental
Life Insurance. This coverage provides an additional benefit
to your beneficiary(ies) if you die. Your personal enrollment
worksheet lists the options available to you, along with
each option's cost per pay period. You pay the cost of
this benefit on an after-tax basis.
Depending on your situation, you may be required to provide
Evidence of Insurability (EOI) when you enroll for Supplemental
Life Insurance coverage, according to the following rules:
- If you are a newly hired employee, you may elect any
coverage level (see below) without providing EOI.
- Current
employees enrolling for the first time must show EOI when
selecting any level of coverage.
- Current employees who
previously enrolled for coverage may increase their coverage
by one level - for example, elect to increase from $10,000
to $20,000 - without providing EOI. EOI will be required
for all increases of more than one coverage level.
- If
you experience a qualified life event or change in status
you may make changes to your Supplemental Life Insurance
coverage that are consistent with and on account of your
change in status. If you are enrolling for the first time
you may elect coverage of $5,000 without providing EOI.
You must show EOI when selecting any other level of coverage
for the first time. If you are already enrolled for coverage
you may increase your coverage by one level without providing
EOI. However, EOI will be required for all increases of
more than one coverage level.
If your Supplemental Life Insurance selection requires you to provide EOI, you
will receive a form from the District's benefits administrator. You will need
to complete and return this form to Standard Insurance Company, as specified
in the information you receive. If your coverage selection is approved, your
coverage will be effective the first of the month following approval and the
appropriate deductions will be taken.
