It’s Time To Enroll
for Your 2008 Health and Welfare
Benefits
The Annual Open Enrollment Period for
medical, dental, vision, and supplemental
life insurance benefit options will
take place Sunday, November 11, 2007
through November 24, 2007 at Midnight,
CST
Personalized confirmation statements
listing your benefits for 2008 will
be mailed to your home in early December.
You will be allowed to
make corrections/changes from December 3,
2007 to December 15, 2007.
SLPS Benefits Representatives are
available to answer your questions
about benefits or the enrollment process
at 1-866-345-SLPS (7577), Monday – Friday,
7 a.m. to 7 p.m., CST except holidays.
Other Information
If you are a newly hired employee,
enroll for
benefits within 30 days of your date
of hire. Employees who change from
a temporary position or laid-off status
to a full-time permanent position are
considered new hires for the purposes
of benefits eligibility. See When

Coverage Begins for more information.
If you choose to opt out of Medical
coverage because you have coverage
under another plan, you will receive
a monthly credit. A credit of $50 per
month will be paid to 12-month employees;
10-month employees receive a $60 monthly
credit. This amount will be included
in the last paycheck of each month,
as taxable wages. If you do not make
any election, you will receive default
benefits as described in your 2007
Enrollment Guide.
Tax-Sheltered Annuities (403b)
Plans – January 2007 Open Enrollment
Period
January 1 through January 31, 2007
is open enrollment for adding or changing
your salary reduction contributions
to tax-sheltered annuities, often referred
to as TSA’s or 403(b) plans. If
you miss this enrollment period, you
will not be able to enroll or change
companies/contributions until the next
enrollment period September 2007.

Creditable Coverage Disclosure Notice

notice has information about your current
prescription drug coverage with The
Board of Education of the City of St.
Louis and prescription drug coverage
available for people with Medicare. It
also explains the options you have
under Medicare prescription drug coverage
and can help you decide whether or
not you want to enroll. At the
end of this notice is information about
where you can get help to make decisions
about your prescription drug coverage.
