HIPPA Form: this form is used to gain authorization from the parent so that the nurse can get pertinent information from the child's physician about their care. The nurse may ask for a signature on this form before requesting information from the child healthcare provider.
OSH 32: this form provides the nurse with information on the child's condition for emergency purposes.
Physical Form: needs to be completed by every student every 2 years.
Sports Physical Form: needs to be completed by any student wanting to participate in sports at SLPS
