Required Documents for the 2024-2025 School Year

    Medications:
    a) Medications provided for the 2023-2024 school year must be picked up by May 23rd, 2024. Any medications not picked up by May 23rd will be disposed of, unless your child is going to summer school at Stix. 
     
    b) Prescription medications (ADHD medication, prescribed eye drops, prescribed ointments) that MUST be given during school; Please have your child’s doctor provide an order to give the medication at school.
     
    c) For Albuterol Inhalers, please provide a doctor’s order AND Asthma Action Plan. Please provide a spacer with your child’s inhaler.
     
    d) For Epinephrine Injection Devices (Epi Pens, Auvi Q), please provide a doctor’s order AND Food Allergy Action Plan.
     
    e) For students needing over-the-counter medication (Tylenol, Motrin, etc…), an over-the counter consent form will need to be completed by the parent. A consent form is provided on request.
     
    f) Please use this link, Doctor's Order Form, to access the form doctors will need to complete for students to receive prescribed medications at school.
     
    g) Doctor’s order, Asthma Action Plan, Food Allergy Action Plan, Over the Counter Medication Form all must be renewed for the 2024-2025 school year. LAST YEARS FORMS CANNOT BE USED.
     
    Food Allergies, Food Intolerances and Special Diets
    Students requiring any modifications to their school provided meals MUST have documentation provided by the doctor. For example, if your child is lactose intolerant, and you would like your child to receive soy milk, your child’s doctor will have to provide documentation of the food intolerance.
     
    Immunizations
    a) All new and returning Pre-K need to provide up-to-date shot records during the registration process.
     
    b) All Kindergartners need to provide up-to-date shot records. Kindergartners are required to have a DTAP, IPV, MMR, and Varicella dated on or after their 4th birthday to be considered compliant with their shots. Call your child’s doctor to determine if your child needs shots for Kindergarten.
     
    c) 1st and 2nd graders who did not go to Stix the prior school year, are required to provide up-to-date shot records.
     
    d) For religious exemptions, please call St. Louis Department of Health at (314) 657-1447. The department of health provides education and documentation of your child's exemption.
     
     
    Physical and Dental Exams
    a) All new and returning Pre-K students need up-to-date lead and hemoglobin. 
     
    b) All Kindergartners need an up-to-date physical exam.
     
    c) All 1st and 2nd graders that are new (2024-2025 is their first school year at Stix) need an up-to-date physical exam. 
     
    d) All students with chronic health issues (Asthma, Food Allergies, Seizures, Diabetes, Sickle Cell, etc...) need an up-to-date physical exam and action plan. 
     
    f) A physical is considered up-to-date if it is less than a year old at registration.
