    The Registration Process - New Pre-K Students 
    It is our goal to make the registration process for new SLPS Parents/Guardians as easy as possible. You are part of our team – in our efforts to provide a quality education for all students in the city of St. Louis. If at any time, during this registration process, you have any questions, please call our office at 314-331-6150. We are here to help!  
     
    You have started the registration process by accessing this website and completing the Pre-registration form.  All you have to do is follow the prompts (buttons) at the bottom of each website page – follow the instructions very carefully – gather required documents, complete online registration forms, then come to our office at 801 N. 11th with all of your required documents and forms, and get ready for the first day of school!
     
    We will be available to register your 
    NEW SLPS student starting February 4th, 2019.

    Our office is avaialable to assist and register families, during the following hours:
     
    Monday:           8:00am  – 4:45pm
    Tuesday:           8:00am  – 4:45pm
    Wednesday:      8:00am  – 4:45pm
    Thursday:         8:00am  – 4:45pm
    Friday:              8:00am  – 4:45pm
     
    No appointment is necessary.  

    To complete the registration process and to best serve you, please have the following REQUIRED DOCUMENTS  at registration:

    1. Official Government Issued Birth Certificate or Passport – Children must be 3 or 4 years old by July 31st of the school year. The birth Certificate must include an official seal. 

    2. Current Immunization/shot Record for your child. (must be valid on the day school starts.)

    3. Current Physical Exam completed by the child's physician. The exam report must include lead and hemoglobin numeric results (within the last year). 

    4. Current Dental Exam completed by the child's dentist (within the last year).
       
    5.  Parent/legal Guardian:

      • Identification: – valid government issued identification or passport. (driver’s license, passport, or state I.D.)

      • Proof of Residency – Must be a current St. Louis city resident with a utility bill in your name and for your present address. The bills have to be recent (within 45 days) and can be: Ameren, Spire Gas, or the St. Louis City Water Division. District employees must also meet residency requirements; condition of employment does not guarantee registration. 
      • Proof of Annual Income – Parents must provide one of the following: Current tax returns or W2; SSI letter or statement; TANF letter; or current Food Stamp Budget Summary that includes your income. 
       

    *If your child has health insurance, please provide proof of coverage.

     Please Note: Health-related documents must be valid on the first day of school. 

    You now have the option to complete registration forms online. By completing these forms, printing them, and bringing them to registration, you will be significantly reducing your time registering in our office.
