• NEW: SD-107 Request Form Usage Criteria 2025-2026 SY

    Greetings SLPS Team! The Facilities Team is thrilled to implement a new streamlined approach to the SD-107 Request Form process as of August 8, 2025. We are aware that there will be updates and adjustments to make as we progress forward in this process. However, the Facilities team is eager to assist in this process to ensure clarity and usability.     In the new process, the SD-107 Request Form must be used ONLY for the transfer or relocation of district-owned equipment that meets specific inventory control thresholds as outlined by SLPS policy and Missouri state regulations. These are items that are typically tracked in the district’s Business Plus system

     

    Items Subject to SD-107 Inventory Tracking Include:

    Tangible property (including IT systems) with a useful life of more than one year and a per-unit acquisition cost of $1,000 or more

    Sensitive or easily pilfered items under $1,000 such as:

    o Audio-visual equipment

    o Digital cameras

    o    Laptops

    o   Televisions

    o Smartboards/Promethean boards

    o iPads/Tablets

    o Power tools

    o Computers and accessories

     

    NEW: Instructions for Completing the SD-107 Request Form:

     

    To transfer or remove any items from your building that fit the outlined criteria above, please complete an SD-107 Form (2025-2026 SY) Standard Delivery Transfer Form. Be sure to include the following:

     

    • Item descriptions, serial numbers, asset tag numbers, and both origin and destination locations

     

    • Ensure all required signatures are provided

     

    • The completed forms will be submitted to the SLPS Inventory Project Technicians for system updates in the financial management database.
